The After Care program offers continued support to all newly housed women including follow-up case coordination and practical assistance services up to 12 months.
All newly housed women are offered the opportunity to apply for follow-up case coordination and practical assistance services from the Wraparound Case Workers leading the Aftercare Program. The Program provides supports for 12 months. All newly housed women are connected with community-based case management and follow-up workers in York Region based on their needs (e.g. Housing Stability Workers; ACT team; OW/ODSP case managers; etc.). Salvation Army staff members negotiate relationships with as many of these community agencies as possible to ensure that referrals will be well received and expectations are clear.
